Please complete the attached Citizenship Affidavit in its entirety. The Citizenship Affidavit must be completed and notarized. A copy of the front and back of the secure and verifiable documentation as indicated on the form must be attached (i.e. Driver’s License, Passport, Permanent Resident Card etc.)Learn more about the Citizenship Verification requirement.
