- Photocopy of GA Business license or GA Occupational license
- Photographs of sign, sales room or office and lot
- Proof of publicly listed telephone number(s) in Georgia
(Cell phones are not accepted.) - Photocopy of proof of a State of GA Tax ID number
- Photocopy of each authorized agent /representative GA driver’s license or GA ID card
- Photocopy of GA Fire Marshal’s license
- Federal Employer Identification number (FEIN)
