• MV-6 Dealer, Distributor & Manufacturer Tag Application
• MV-6A Authorize / Add / Delete Agents
• MV-6B Dealer, Distributor, Manufacturer & Transporter Application for Additional (2 or more) Tags
  (Signature must be notarized).
• MV-6C Dealer, Distributor, Manufacturer & Transporter, Renewal Application
• Photocopy of GA Business license or GA Occupational license
• Photographs of sign, sales room or office and lot
• Proof of publicly listed telephone number(s) in Georgia
  (Cell phones are not accepted.)
• Photocopy of proof of a State of GA Tax ID number
• Photocopy of GA Sales Tax Certificate
• Photocopy of each authorized agent /representative GA driver’s license or GA ID card
