- Complete purchaser information using their full legal name as it appears on their Georgia Driver’s License or Georgia Identification Card.
- Purchaser’s current address.
- Date of sale or ownership transfer (month, day, and year).
- Complete the odometer reading at the time of the sale in complete miles, do not include 10ths of a mile.
- Check the appropriate box for:
- Odometer reading is in excess of its mechanical limits and the actual miles is more than:
- 99,999 on a 5-digit odometer
- 999,999 on a 6-digit odometer
- Odometer reading is NOT the Actual mileage because the odometer:
- Is broken, replaced, or repaired
- Does not reflect the actual miles
- Odometer reading is in excess of its mechanical limits and the actual miles is more than:
- Print and sign your name under seller. If jointly owned, each seller must print and sign.
- If the seller is a company, the printed name of the company is required in addition to the printed names and signature of the company’s authorized agent. The company’s authorized agent should enter his or her position with the company after the signature.
- The buyer should print and sign their name under Buyer. If jointly owned, each buyer must print and sign.
