Consumption on Premises

On this page, find guidelines for completing the Consumption on Premise License application online using the Georgia Tax Center.

Once your online submission is processed, you will receive a notification containing specific account information.

All applicants applying for a Consumption on Premise License must provide background and fingerprint process approved by DOR.

Required Forms & Documents

• Citizenship Verification - Alcohol, Tobacco, and Motor Fuel Affidavit for Citizenship Verification
• Form ATT-17 - Georgia Alcohol & Tobacco Personnel Statement (requires notarization) 
• Financial Affidavit
• RD-1061 - Power of Attorney and Declaration of Representative (if applicable)

License Description

Consumption on Premises -  A Retail Consumption dealer (person/business) sells distilled spirits, malt beverage and/or wine for consumption on the premises. Retail sales only to consumers and not for resale.
