Accepted payments: Money order, cashier's/certified check, or cash. No personal checks will be accepted.

Unclaimed Plates

Copy of applicant's Georgia Driver's License or ID
Power of Attorney (if not owner)

Handicap Placards

Completed / notarized Form MV-9D by Georgia licensed physician (if receiving for the first time).
Expired placard, if renewing
