Replace Missing/Damaged Serial Plate
On this page find information on replacing missing or damaged serial plates. You will need to visit your County Tag Office or apply by mail to replace missing or damaged serial plates:
Gather Required Documents
- Completed and signed MV-1 Title/Tag Application.
- Save Time! Start your MV-1 Motor Vehicle Title/Tag Application online BEFORE you visit the county tag office.
- If it is known, the MV-1 should contain the original serial number of the vehicle
- Original valid Title or Manufacturer’s Certificate of Origin
- Damaged serial plate, if you have it
- A picture of the vehicle is required if vehicle is 1985 or older with no previous Georgia record
- Completed and signed T-128 Missing Serial Plate Affidavit
- Completed and signed T-22B Certificate of Inspection
- $18 application fee (if there is a change to the title) and $5 serial plate fee
Next Steps
Once the information is received and processed, the Department will mail you:
- Replacement serial plate
- Partially completed Form T-22 Certificate of Inspection for assigned serial plate or T-88 for a rebuilt serial plate
After receiving the serial plate and Form T-22 or T-88, you need to:
- Attach the replacement serial plate to your vehicle with rivets
- Have a Georgia law enforcement officer complete and sign the T-22 or T-88 certifying that the replacement serial plate has been properly affixed to your vehicle
- Mail the completed T-22 or T-88 to the MVD at the address below. The MVD will issue a new title.
