- Funds that have been taken will be applied to the balance of your account under lien.
- You may lose your right to claim an exemption if you do not file your claim form within 20 days after receiving this notice.

- Once you submit the exemption form, the State will review your claim within 10 days of receipt and will send the decision to the contact information you provided in the letter.

Notice to Taxpayer of Right against Levy of Money and/or Other Property (Letter)
You have received this notice because money and/or other property belonging to you has been levied/taken by the Georgia Department of Revenue to pay the State Tax Execution(s), also called liens, against you.
This letter informs you of any funds that are not subject to levy garnishments and how you claim these funds back, if applicable.
Your Letter Explained
- The money taken out of your bank account might have included funds that are sourced from entities like social security, unemployment, Veteran’s Administration, disability, or more. More information available from the Attorney General.
- The letter/form provides you with a way to claim an exemption on these funds.
- Complete the form with all requested information and fax or mail to the address and number listed on the letter, within 20 days after receiving this notice to be reviewed.
