Importer (In-State)

Once your online submission is processed, you will receive a notification containing specific account information.

Required Forms & Documents

• Malt Beverage Bond Performance & Tax Liability Bond
• Wine Performance & Tax Liability Bond
• Liquor Broker Bond
• Form ATT-17 - Georgia Alcohol & Tobacco Personnel Statement (requires notarization)
• Scanned copy of current annual registration from the GA Secretary of State
• Citizenship Affidavit along with secure and verifiable documents, i.e. government issued photo ID (requires notarization)
• Scanned copy of Federal Basic Permit
• Scanned copy of Local License
• Scanned copy of Lease or Deed
• Form ATT-104 - Brand & Label Registration and Designation of Sales Territory
• Power of Attorney (if applicable)
• Financial Affidavit

Importer - An Importer brings an alcoholic beverage into Georgia from a foreign country to sell to another importer, broker or wholesaler who keeps a stock.
