Important note regarding pages to submit

Beginning Tax Year 2023, when applying for a Net Operating Loss on Form 500, Schedule 4 must be submitted with the completed Form 500 pages 1-5, as well as any other applicable Schedules 1-3 and IND-CRs.

If Form 500 Schedule 4 is submitted without the required Form 500 and any other applicable pages, the Schedule 4 will not be processed.
