You are encouraged to make an appointment to help minimize wait times and maintain the safety of our staff and customers. To make an appointment, please complete the Customer Service Appointment Form below.

Once your form is submitted, the Department will contact you within 3 hours
Forms received after 4:00 pm will be processed on the next business day.
You will receive a confirmation email with your appointed time.

For your convenience a secure lockbox is available 7:30 am to 4:30 pm at DOR Headquarters, 1800 Century Boulevard, Atlanta, GA 30345.

Driver’s license or driving history reports (provided at DDS)
Payment of Title Ad Valorem Tax (provided at local tag office)
Payment of insurance penalties and fines (provided at local tag office)
